The #EIS Study, implemented by CHAK Kenya and supported by the Gates Foundation, is generating key evidence on #integrating #HIV, #Diabetes, and #Hypertension care across 50 facilities in Nairobi, Kitui, and Kiambu Counties. The study pilot is guided by #IMARA (Integrated Model for Accessible, Resilient and Accountable Care package), a locally driven evidence-based framework for delivering integrated HIV and Non-Communicable Diseases (NCD) care in primary health settings designed to support counties, partners, and health facilities to adopt, implement, and sustain person-centered, co-managed chronic care models across Kenya.
